I hereby confirm that all individuals listed on this form are, to the best of my knowledge, residents in the UK or areas covered by the organization at the time of completing the form. I understand that this information must be true and correct, and I acknowledge that the organization reserves the right to verify its accuracy.

Furthermore, I agree to abide by the decision of the organization in the event that the above information is found to be incorrect. Additionally, I consent to refund MNBF any amount received by me should I choose to relinquish my membership within 3 years of receiving any financial assistance. I understand that MNBF retains the right to pursue the refund through legal means if necessary.
